Presentation
Abstract The presentation of two cases of veno-occlusive priapism, in patients of 36 and 58 years, in whom the different medical and surgical techniques employed, failed. [nature.com]
C Discuss the immediate implantation of a penile prosthesis with the patient in cases of priapism presenting > 36 hours after onset, or in cases for which all other interventions have failed. [uroweb.org]

Entire Body System
- Pain
The patient typically reports an erection that is not fully rigid and is not associated with pain although fully rigid erections may occur with sexual stimulation. [uroweb.org]
High-flow priapism generally follows a perineal trauma and, as it is nonischemic, it does not cause pain, neither does it generate fibroses nor lead to impotence. [nature.com]
Stuttering (intermittent) priapism is a recurrent form of ischemic priapism in which unwanted painful erections occur repeatedly with intervening periods of detumescence. [auanet.org]

- Surgical Procedure
C Perform distal shunt surgical procedures first followed by proximal procedures in case of failure. [uroweb.org]
Treatment
Objective: To explore the diagnosis and treatment of veno-occlusive priapism. Methods: The diagnosis and treatment of 17 cases of veno-occlusive priapism were analyzed retrospectively. [pubmed.ncbi.nlm.nih.gov]
It should be considered in the differential diagnosis of those patients with veno-occlusive priapism, in whom treatment fails. [nature.com]

Additional tables detailing side effects were developed for some treatments. These results were then summed to provide crude estimates of treatment effects. [auanet.org]
B Disease Management The treatment is sequential and the physician should move on to the next stage if the treatment fails. Figure 1: Treatment of ischaemic priapism * The dose of phenylephrine should be reduced in children. [uroweb.org]
Etiology
While the etiology of the recurrent ischemic priapism is often idiopathic, patients with hematologic abnormalities, such as sickle cell disease, are more prone to developing recurrent (stuttering) priapism. [auanet.org]

Prevention
This pattern of recurrence, known as stuttering priapism, challenges the clinician to develop a management strategy to prevent future episodes of priapism. [auanet.org]
B Use hormonal therapies (mainly gonadotropin-receptor hormone agonists or antagonists) and/or antiandrogens for the prevention of future episodes in patients with frequent relapses. Do not use them before sexual maturation is reached. [uroweb.org]
