Presentation
We present a case of this syndrome in a 31-year-old male who presented with a diffuse erythematous morbilliform rash with high fever and elevated liver enzymes. [cureus.com]
The classic clinical presentation of vesicles progressing to painful ulcers is unusual; atypical and mild symptoms are common, and most people have unrecognised disease. [bestpractice.bmj.com]
The clinical presentation depends on portal of entry, age, immune status and type of HSV (1 or 2) infection. [health.vic.gov.au]

Entire Body System
- Fever
The most common sign is fever ≥ 38.5 °C, followed by lymphadenopathy. [cureus.com]
About 10 per cent of primary infections cause a more severe form of disease manifested by fever and malaise. [health.vic.gov.au]
A person may experience swelling, shooting or sharp pains in the breasts or chest, and a fever. Mastitis is common during breast-feeding. The infection may clear up on its own, though some people require antibiotics or a hospital stay. [medicalnewstoday.com]
Skin
- Dermatitis
The biopsy revealed spongiotic dermatitis with subcorneal pustules along with superficial perivascular and mixed lymphocytic and neutrophilic infiltrate with dermal edema and rare eosinophils. [cureus.com]
In children with atopic dermatitis/eczema and immunosuppressed patients, HSV may cause a generalised eruption that requires hospitalisation for antiviral therapy. [health.vic.gov.au]
Neurologic
- Burning Sensation
For example: Acid reflux can cause a burning sensation in the chest. Gallstones can cause sudden, intense pain that lasts for several hours, disappears, and returns. Ulcers can cause pain that comes and goes. [medicalnewstoday.com]
Workup
Workup during that hospitalization revealed psychogenic seizures and Keppra and Dilantin were both discontinued. [cureus.com]
Treatment
Treatment is mainly stopping the culprit drug and starting systemic steroids based on the severity of the disease. Prompt diagnosis and treatment of DRESS syndrome are critical due to its high morbidity and mortality. [cureus.com]
Control of case Nongenital herpes For symptomatic treatment of minor attacks, use povidone iodine 10 per cent paint applied three times daily, and/or topical antiviral therapy. [health.vic.gov.au]

Etiology
Its etiology has been linked with lymphocyte activation, drug metabolic enzyme defects, eosinophilia, and human herpesvirus-6 reactivation [2]. [cureus.com]
